Apply for an assessment online in 3 easy steps
Before you can access government-subsidised aged care services for the first time, you need to apply for an assessment.
Using the online application is quick and easy and will only take 15 to 20 minutes to complete.
It’s the same form to apply for all types of care and support – including help at home, short-term care, and care in an aged care home.
You can complete the application yourself or on behalf of a family member or friend. If you’d like to set up an ongoing representative, you can also do that here.
Part A: Am I eligible?
Eligibility is based on need. To find out if you meet the criteria, we will ask a few questions about your current situation and if you need help with some everyday tasks. This step will take about 5 minutes.
Part B: Provide your details
Here you will be asked for some information about yourself. You’ll need your Medicare card to complete this step.
If you are applying on behalf of someone else or setting up a representative, you will need to provide your information as well as theirs. This step will take about 10 minutes.
Part C: Arrange the assessment
To finish, we will ask you who the assessor should contact to arrange the face-to-face assessment. This step will take about 5 minutes.
This is a new online form that makes it easier to find out if you are eligible and apply for a face-to-face assessment. If you need an assessment, please try this new service and let us know what you think of it through the survey at the end.
